When to get help
Context and scope

There is no single missed-dose rule for GLP-1 medicines: each label sets its own, and the same medicine can be worded differently in the United States and in Europe. This article lays the rules side by side as the product information states them, explains the two ways the weekly labels count, sets out when a longer gap means starting again at a lower dose, and lists what to have ready when you ask your pharmacist.

Key takeaways 4
  • The same medicine can be worded differently by region: the EU Wegovy® document uses a 5-day window, while the US label asks whether the next dose is more than 2 days away. [2][11]
  • For daily liraglutide, the US Saxenda® and Victoza® labels restart at 0.6 mg after more than 3 days without a dose; the EU Saxenda® document allows a single late dose only within 12 hours. [7][8][13]
  • Restart thresholds differ too: 2 or more weekly Wegovy® injections in a row, more than 3 days of liraglutide, 7 or more Foundayo™ tablets in a row. [2][7][8][9]
  • The Ozempic®, Mounjaro®, Zepbound® and Trulicity® labels close each missed-dose rule the same way: afterwards, dosing returns to the regular once-weekly schedule. [1][4][5][6]

Two ways of counting, side by side

The weekly labels use one of two clocks. Some count forward from the day the dose was due and give a window: inside it the late dose is given, beyond it the dose is dropped. Others count backwards from the next planned dose: if enough time is left, the late dose goes in; if not, it is dropped. Daily tablets of semaglutide use neither; the missed tablet is simply skipped.

The table collects the rule from each US label with its section number, and adds the European or Canadian wording where it differs or confirms it. It summarises documents for comparison. The leaflet in your own box, and what your prescriber has told you, are what apply to you.

Daily tablets and daily injections

  • Wegovy® tablets: the missed tablet is not made up; the next one is taken the following day [2].
  • Rybelsus® and Ozempic® tablets: the same skip-to-tomorrow rule, and the label adds that no more than one tablet is taken in a day [3]. These tablets are taken on an empty stomach in the morning with a little water, and the label offers no later-in-the-day option: it describes moving on to the next day.
  • Saxenda® and Victoza® (liraglutide): no extra dose and no larger dose to make up; the routine carries on with the next scheduled injection [7][8]. The Victoza® Medication Guide adds plainly that two doses are not taken at the same time [8].
  • Foundayo™ (orforglipron): the missed tablet is taken as soon as possible, and the label says not to double up the next one [9].

When the gap is longer than one dose

A longer break is handled differently from a single late dose. Starting again carries its own risk of stomach side effects, so the labels that address it restart at a lower dose [2][7][9]:

  • Wegovy® injection: after 2 or more missed weekly doses in a row, dose escalation starts again at a lower dose. The Medication Guide turns this into a call to your healthcare provider about how to restart [2].
  • Saxenda® and Victoza®: if more than 3 days have passed since the last dose, treatment restarts at 0.6 mg a day; Saxenda® then follows its escalation table, and Victoza® is titrated as the prescriber decides [7][8].
  • Foundayo™: after 7 or more missed tablets in a row, escalation restarts at a lower dose [9].
  • EU Wegovy® document: if more doses are missed, a lower starting dose for re-initiation should be considered [11].
  • The US Ozempic®, Mounjaro®, Zepbound® and Trulicity® labels do not set a number of missed doses that triggers a restart [1][4][5][6]. That is a reason to ask, not a reason to assume the old dose is fine.

Same medicine, different documents

Product information is approved country by country, and the wording on missed doses does not always match. The clearest example is semaglutide for weight management: the US Wegovy® label uses the 48-hour look-ahead rule, while the EU summary of product characteristics and its leaflet allow a missed injection within 5 days, as for Ozempic® [2][11].

Liraglutide differs as well. The US labels say only that the next dose is taken as scheduled, with a restart after more than 3 days, whereas the EU Saxenda® document and the EU Victoza® leaflet allow a late dose within 12 hours and otherwise skip to the next day [7][8][13][14]. Day changes vary too: the US Ozempic® label requires more than 48 hours between two doses, the EU Ozempic® document at least 3 days [1][10]. If you have moved country, or have a pack from another market, the leaflet in that pack is the one to read, and a pharmacist can reconcile the two.

After a late dose: back to the usual schedule

The Ozempic®, Mounjaro®, Zepbound® and Trulicity® labels end their missed-dose instructions the same way: once the late dose is given or dropped, dosing returns to the regular once-weekly schedule [1][4][5][6]. Wegovy® says to resume on the usual weekday when the dose is dropped [2].

A late dose can leave the next one closer than usual. The labels set minimum gaps when a weekly day is changed: more than 48 hours for Ozempic® and at least 2 days for Wegovy®, at least 72 hours for Mounjaro®, Zepbound® and Trulicity® [1][2][4][5][6]. If a late dose and the next planned one look close together, that is a good moment to ask your pharmacist which day to use. Moving the weekly day on purpose has its own article.

If you doubled up or took too much

None of the labels describes taking two doses close together to catch up, and several rule it out directly: Saxenda®, Victoza® and Foundayo™ in the US, and the EU leaflets, which say not to use a double dose [7][8][9][10][11].

If you think you took too much, the Wegovy® Medication Guide warns that severe nausea, severe vomiting and severe low blood sugar can follow, and lists three options: phone your healthcare provider, phone Poison Help (1-800-222-1222), or head to the closest emergency department without waiting [2]. The Ozempic® label adds that observation may need to last longer because semaglutide has a half-life of about a week [1]. Outside the United States, use your local poison information or emergency number.

What to have ready when you ask

Because the rules are counted in hours and days, the most useful facts are exact: the date and time of the last dose you actually took, the strength, how many doses were missed in a row, which step of the dose schedule you were on and any symptoms since. If you take insulin or a sulfonylurea, mention it as well, since the labels link those medicines to low blood sugar. With that in hand, a pharmacist can match your situation to the label for your product in a few minutes.

Questions

I remembered my Ozempic® injection on day 6. Can I still take it?

The US label allows a missed injection within 5 days of the day it was due. After that, it says to drop the missed dose and give the next one on the regularly scheduled day [1]. Your pharmacist can confirm this against your own pack.

Why does my European Wegovy® leaflet say 5 days when the US label talks about 2 days?

The documents were approved separately. The US label asks whether the next dose is more than 2 days away, while the EU summary of product characteristics and leaflet use a 5-day window from the missed day [2][11]. Follow the leaflet that came with your medicine and check with a pharmacist if you have both.

My Trulicity® dose is due tomorrow and I just found the missed one. What does the label say?

With fewer than 3 days (72 hours) left before the next dose, the Trulicity® label says not to give the missed one and to carry on with the next scheduled dose [6].

I help my father with Victoza® and he missed four days. Does he just restart?

The US label says that after more than 3 days without a dose, Victoza® is restarted at 0.6 mg a day and then titrated at the healthcare provider’s discretion [8]. Call his prescriber before the next injection.

I remembered my Rybelsus® tablet after breakfast. Can I take it now?

The label says a missed tablet is skipped and the next one taken the following day, and that no more than one tablet is taken per day [3]. The tablets are meant for an empty stomach in the morning, so the next morning is when the label resumes.

Can I take two Foundayo™ tablets tomorrow to catch up?

No. The label says to take a missed tablet as soon as possible but not to double up the next dose, and to restart escalation at a lower dose after 7 or more missed tablets in a row [9].
