Context and scope

A change in breath is one of those side effects people notice in others before themselves, or hear about from a partner. Wegovy® documents never mention it by name. What they do record are the stomach effects that send air, acid and food smells back up. This page connects those label figures to the main causes of bad breath, and flags the two breath smells that point to an emergency.

Key takeaways 5
  • No US, EU or Canadian Wegovy® document lists bad breath or halitosis. [1][2][3]
  • Burping: 7% on Wegovy® vs under 1% on placebo in the US table; 7.4% vs 0.4% in Canada; 4% vs 0% in adolescents. [1][3]
  • Reflux (5.4% vs 3.0%), indigestion (9.0% vs 3.2%) and vomiting (24% vs 6.3%) were all more common on Wegovy® in Canada’s trial pool. [3]
  • A 2026 review describes “Ozempic breath” as a patient-reported complaint tied to nausea, vomiting, belching and lower fluid intake, with scarce human data. [4]
  • MedlinePlus says breath that smells like stool can occur with prolonged vomiting, especially with a bowel obstruction. [5]

What the Wegovy® tables record near the mouth

We searched the three Wegovy® documents for bad breath, halitosis and breath odour and found no entry. [1][2][3] The rows below are the reactions that most plausibly change how breath smells.

Timing: the dose-escalation months

The US label says its gastrointestinal reactions were reported most often during dose escalation. [1] The EU summary marks burping, reflux and indigestion as mainly seen in that period and reports a median duration of 8 days for nausea and 2 days for vomiting. [2] If your breath changed at the same time as burping or reflux, it may follow the same pattern across dose steps, although no study has tracked breath directly.

Severe stomach reactions were reported in 4.1% of adults on Wegovy® injection against 0.9% on placebo, and the label does not recommend Wegovy® for people with severe gastroparesis. [1]

Three ways Wegovy® could change your breath

  • From the stomach upwards (label-supported): burping and reflux bring gas and acid back into the mouth, and Cleveland Clinic names reflux among common causes of bad breath, describing a sour taste and smell. [1][6]
  • From eating very little (proposed, not measured): Cleveland Clinic explains that ketosis develops with fasting or a very low-carbohydrate diet and lists “keto breath” among its effects. [7] Whether people on Wegovy® commonly reach ketosis has not been studied.
  • From a drier mouth (proposed): a 2026 review links nausea, vomiting, belching and reduced fluid intake to complaints labelled “Ozempic breath” and “Ozempic mouth”, noting that much of the evidence is indirect. [4] Saliva rinses the mouth, and too little of it is a recognised cause of halitosis. [6]

Most bad breath starts in the mouth

MedlinePlus says bad breath is usually related to dental hygiene: bacteria left by irregular brushing and flossing release sulfur compounds. [5] It lists cavities, abscessed or impacted teeth, gum disease, sinus and throat infections, smoking, alcohol use, coffee, some foods such as garlic and onion, and high-dose vitamin supplements among common causes. [5]

Cleveland Clinic adds tonsil stones, infections in the nose, throat or lungs, diabetes through gum disease, and kidney or liver disease. [6] Because several of these are common regardless of medicine, a dental check is a sensible first step if the odour does not settle with the stomach effects.

Two breath smells that are warning signs

Fruity breath: MedlinePlus describes a fruity odour as a sign of ketoacidosis, a potentially life-threatening problem in people with diabetes. [5] Cleveland Clinic separates this from ordinary ketosis, which does not make the blood acidic. [7] Wegovy® is used by some adults with type 2 diabetes, and the Canadian patient information asks you to tell your doctor if you have ever had diabetic ketoacidosis. [3]

Stool-like breath: this can occur with prolonged vomiting, especially when the bowel is blocked. [5] Bowel obstruction, ileus and severe constipation with impaction are among the reports received for semaglutide after approval. [1] MedlinePlus lists keeping on vomiting, a swollen belly that does not go down and being unable to pass stool or gas as reasons to contact a provider about a possible blockage. [8]

What general sources suggest

Cleveland Clinic’s prevention advice covers brushing twice and flossing once a day, cleaning the tongue, an alcohol-free antibacterial mouthwash, regular dental cleanings, drinking water to prevent dry mouth, sugar-free gum or mints to boost saliva, and avoiding alcohol, caffeine and tobacco. [6] It notes that some mouthwashes only mask odour. [6]

For the stomach side, the Wegovy® Medication Guide asks you to report severe or lasting stomach problems to your provider. [1] If burping or reflux is the likely source, that conversation is more useful than mouth products alone.

Questions

Does Wegovy® cause bad breath?

It is not listed in the US, EU or Canadian documents. [1][2][3] Burping, reflux, indigestion and vomiting are, and each can affect breath. [3][6]

Why might my breath smell like sulfur on Wegovy®?

The documents record burping but not its smell. [1] MedlinePlus explains that mouth bacteria release sulfur compounds, so cleaning teeth and tongue is part of the answer either way. [5]

Is “keto breath” a Wegovy® side effect?

Not as a listed effect. Cleveland Clinic links keto breath to ketosis from fasting or very low-carbohydrate eating. [7] Whether this happens on Wegovy® has not been studied.

Does bad breath on Wegovy® go away?

There are no data on breath itself. The related stomach reactions are most frequent during dose escalation, and the EU summary reports short median durations for nausea and vomiting. [1][2]

My teenager on Wegovy® has bad breath. Is that expected?

Bad breath is not listed for adolescents. Burping (4% vs 0%) and vomiting (36% vs 10%) were more common on Wegovy® in the teen trial. [1] A dental check rules out mouth causes. [6]

Will mouthwash fix it?

Cleveland Clinic notes that some mouthwashes only mask odour and suggests an alcohol-free antibacterial type alongside brushing, flossing and tongue cleaning. [6] If reflux is the cause, it needs its own attention.
