What the Wegovy® documents contain
Searching the full US label, EU product information and Canadian monograph for libido, erectile and sexual function turns up animal data only: semaglutide delayed sexual maturation in young rats of both sexes, without later effects on fertility. [2][3] In adolescents treated for 68 weeks, Canada reports no clinically relevant differences in pubertal development. [3] Several label sections touch on sexual health indirectly.

| Erectile dysfunction, low libido, orgasm problems | Not listed in adults, adolescents or post-marketing reports | US, EU and Canada [1][2][3] |
| Fatigue (includes weakness) | 11% on Wegovy® vs 5% on placebo, adults | US Table 3 [1] |
| Low blood pressure reactions | 1.3% vs 0.4%; more frequent with blood pressure medicines | US section 6.1 [1] |
| Contraception | Recommended for women who could become pregnant | EU section 4.6 [2] |
| Pill hormones | No clinically significant change in ethinyl estradiol or levonorgestrel levels with semaglutide | US section 12.3 [1] |

The study closest to Wegovy® users
Because Wegovy® is mostly used without diabetes, the most relevant study is a TriNetX health-record analysis of men aged 18 to 50 with a BMI above 30, no diabetes and no prior erectile problems or testosterone treatment. Men prescribed semaglutide after June 2021 were matched one-to-one with men who were not; their average age was about 38. [4]
A new erectile dysfunction diagnosis or prescription for an erection pill appeared in 1.47% of the semaglutide group and 0.32% of the comparison group, and new low testosterone diagnoses in 1.53% and 0.80%. [4] The study could not tell whether this came from the medicine or from differences in how often each group saw doctors, and it did not separate Wegovy® from other semaglutide products. The absolute numbers are small on both sides.
Studies pointing the other way
A 2026 meta-analysis pooled seven studies with 237 men on GLP-1 medicines. Erectile function scores improved, and total testosterone and sex hormone–binding globulin rose, while free testosterone did not change significantly. [5] The authors called for larger randomised trials.
A 2026 review of men and women described animal studies in which GLP-1 activation in reward areas of the brain reduced sexual behaviour, but found human data mixed: adverse event reports mention low desire and erectile or orgasm problems without a clear signal, and a randomised trial in healthy men showed no change in desire. [6] Evidence from people taking Wegovy® 2.4 mg specifically is not available.
Women: an evidence gap
A 2026 review from Mayo Clinic authors focused on midlife women. It noted that obesity impairs sexual function through hormonal, vascular and psychological routes, and that weight loss through lifestyle change or surgery has improved it. For GLP-1 medicines, it described possible dampening of desire and arousal through brain reward pathways, supported by case reports and animal work, but very limited systematic evidence. [7] It suggested that clinicians ask about sexual function before and during treatment.
In the 68-week weight trials, 71% of participants were women, [3] yet sexual outcomes were not reported as adverse reactions.
Why sexual function might change on Wegovy®
- Label-supported: fatigue and stomach reactions are common in the early months, [1] and feeling unwell rarely helps desire.
- Label-supported: low blood pressure reactions occurred more often on Wegovy®, especially with blood pressure medicines, [1] and blood flow is central to erections. [8]
- Research-supported: weight loss is associated with higher testosterone in men, [5] and MedlinePlus lists losing extra weight among lifestyle changes that help erection problems. [8]
- Proposed: animal studies suggest GLP-1 signalling in the brain may reduce sexual motivation; this has not been confirmed in people. [6]
- General: mood, body image, stress and relationship changes all influence sex, [8][9] and can shift quickly during noticeable weight change.
Other causes to rule out
MedlinePlus lists common physical causes of erection problems: diabetes, high blood pressure, heart or thyroid conditions, narrowed arteries, depression, low testosterone, nerve damage and nicotine, alcohol or cocaine use, as well as antidepressants, blood pressure medicines (especially beta-blockers), digoxin and sleeping pills. [8] For women, it points to diabetes, heart disease, nerve disorders, hormone problems, some drugs, stress, depression and relationship concerns. [9]
Many people starting Wegovy® already have one of these conditions or medicines, so a change is worth reviewing in full rather than blaming the newest prescription. [8]
Points to raise with your prescriber
MedlinePlus advises seeing a provider when erection problems happen more than a quarter of the time, and for women, when problems last more than a few months or cause distress. [8][9] It warns that erection pills must not be combined with nitroglycerin, and may not suit people with very low blood pressure or some heart conditions. [8] Since Wegovy® can lower blood pressure, mention any dizziness or fainting when this is discussed. [1]
If periods become more regular or fertility returns as weight falls, the pregnancy rules matter: the Medication Guide asks you to stop Wegovy® 2 months before a planned pregnancy. [1]
