Context and scope

Of the side effects listed for Wegovy®, hair loss is the one with the clearest tablet-specific data, and it is higher than the figure many people have seen quoted for the injection. This page explains where the tablet number comes from, why it differs from the injection figure, what the trial says about severity and recovery, how weight loss fits in, what else can make hair thin, and what general health sources describe.

Key takeaways 5
  • EU product information for the tablet: hair loss in 6.4% on oral semaglutide 25 mg and 2.0% on placebo in OASIS 4; all mild or moderate, and half had recovered by the end of the trial. [2]
  • The trial’s public results show the same split as counts: 13 of 204 people on the tablet and 2 of 102 on placebo. [3]
  • Hair loss was reported more often in people who lost 20% or more of their body weight. [2]
  • The US label’s hair loss numbers come from injection studies and link the reaction with weight reduction; in those studies women reported it far more often than men. [1]
  • Shedding after a physical stress such as rapid weight loss usually starts a few months later and is usually temporary. [6][7]

Hair loss in the tablet documents

Unlike most side effects of the tablet, hair loss has its own tablet figure in an official product document. The EU summary for Wegovy® tablets reports it from OASIS 4, the 64-week trial of the 25 mg tablet, and the sponsor’s posted results give the underlying counts. [2][3] The US label, which covers both forms, describes hair loss only from injection trials and states that the reactions in that part of section 6.1 apply to the injection and the tablet alike. [1]

Why the tablet figure is higher than the injection one

The 6.4% from the tablet trial and the 3.3% from the injection trials are not a head-to-head comparison. They come from different studies with different people, and no trial has tested the tablet against the injection. [1][2] Two features of the tablet study are worth knowing when you read the numbers side by side.

First, the tablet trial was mostly women: 76% of the people who took part in the safety analysis were female. [1] In the injection data, where the label does split by sex, hair loss was reported by 4% of women and 0.9% of men on 2.4 mg, and by 8.4% of women against 0.2% of men on 7.2 mg. [1] The tablet documents give no split by sex, so it is not known how much of the higher tablet figure this explains.

Second, weight loss. In OASIS 4, average weight fell by 13.6% on the tablet against 2.2% on placebo at week 64, [4] and the EU summary reports hair loss more often in those who lost at least 20%. [2] A larger study of oral semaglutide at 50 mg once daily, OASIS 1, shows a similar picture in its posted results: alopecia in 6.9% against 2.7% on placebo. [5] That strength is not one of the Wegovy® tablet strengths in the US or EU documents (1.5, 4, 9 and 25 mg), so treat it only as supporting background. [1][2]

Severity, timing and recovery

The EU summary is reassuring on severity: no tablet case was rated severe, and half had recovered by the end of the trial, which ran for 64 weeks of treatment plus a seven-week follow-up. [1][2] The documents do not say when during the trial the shedding began, or how long it lasted in those who recovered.

For shedding triggered by a physical or emotional stress in general, known as telogen effluvium, MedlinePlus says you may not notice it for weeks to months after the trigger and that it decreases over six to eight months; it is usually temporary but can become long-term. [6] Cleveland Clinic places the start two to three months after the stressor, says that most short-lived cases resolve, and describes regrowth after the shedding phase. [7] These are general descriptions and were not measured in the tablet trial.

How weight loss and hair may be linked

  • What the documents support: the US label links hair loss with weight reduction, and the EU summary found it more often with a loss of 20% or more. [1][2]
  • A recognised general pathway: MedlinePlus lists crash diets, especially those short on protein, among the causes of telogen effluvium, and Cleveland Clinic names rapid weight loss and restrictive diets as triggers. [6][7] Neither source studied semaglutide.
  • What is not shown: no Wegovy® document proposes a direct effect of semaglutide on hair follicles, and the trial data cannot separate the medicine from the weight loss it causes.

Other reasons hair can thin

MedlinePlus lists inherited pattern thinning, which increases with age, and shedding after childbirth, major surgery or illness, high fever or severe emotional stress. Some medicines can contribute, including retinoids, birth control pills, beta-blockers, calcium channel blockers, certain antidepressants and NSAIDs such as ibuprofen. [6]

Patchy or unusual loss suggests other causes: alopecia areata, anaemia, thyroid disease, hormone changes, autoimmune conditions, ringworm of the scalp, or hairstyles that pull hard on the roots. [6] These are reasons a clinician may want to examine the scalp or run blood tests rather than assume the tablet is responsible.

What general sources describe

For hair loss linked to illness or a medicine, MedlinePlus says no treatment is usually needed and hair generally grows back once the cause has passed; a wig, hat or new hairstyle can help in the meantime. [6] Cleveland Clinic stresses finding and addressing the underlying stressor, eating enough protein and avoiding extreme or restrictive diets, and asks you to check with a healthcare provider before starting any supplement. [7]

Two tablet-specific points apply to supplements. The Medication Guide asks you to tell your provider about all vitamins and herbal supplements, because Wegovy® slows stomach emptying, and the label asks for at least 30 minutes after the tablet before taking other oral medicines. [1] Your pharmacist can explain how a supplement fits into that morning routine.

Questions

How common is hair loss on Wegovy® tablets?

In the OASIS 4 trial of the 25 mg tablet, 6.4% on the tablet and 2.0% on placebo reported hair loss. [2][3] The EU product information classes it as common.

Why is the Wegovy® tablet figure higher than the 3% I have seen for Wegovy®?

The 3.3% comes from injection trials of 2.4 mg, the 6.4% from the tablet trial. [1][2] They are separate studies; the tablet trial was 76% women, and in the injection data women reported hair loss far more often than men. [1]

Does hair grow back on Wegovy® tablets?

In the tablet trial, half of the hair loss cases had recovered by the end of the study, and all were mild or moderate. [2] Stress-related shedding in general is usually temporary. [6][7]

Is hair loss on Wegovy® tablets linked to how much weight I lose?

The EU summary says it was reported more often in people who lost 20% or more of their body weight, and the US label associates hair loss with weight reduction. [1][2]

Does oral semaglutide 50 mg show the same pattern?

In the OASIS 1 trial of 50 mg once daily, the posted results list alopecia in 6.9% against 2.7% on placebo. [5] That is not a Wegovy® tablet strength in the US or EU documents. [1][2]

Can I take a hair supplement with my Wegovy® tablet?

Not with the dose itself: other oral medicines wait at least 30 minutes, and the Medication Guide asks you to report all supplements. [1] Cleveland Clinic advises checking with a provider before adding one. [7]
