Context and scope

A dry, sticky mouth is easy to overlook, yet it can be a small early clue that you are not drinking enough, especially in the weeks when a GLP-1 medicine is upsetting your stomach. Foundayo™ does not list dry mouth as a side effect, but its label does carry a warning about fluid loss and the kidneys. This page explains how those connect, what the trial registries recorded, and what the NIH and MedlinePlus suggest for a dry mouth itself.

Key takeaways 4
  • Neither the US Prescribing Information nor the Medication Guide mentions dry mouth [1][2].
  • Acute kidney injury was reported in 0.2% on Foundayo™ against 0.05% on placebo, mostly linked to fluid loss from stomach side effects [1].
  • MedlinePlus lists a dry or sticky mouth and thirst among the first signs of dehydration [9].
  • Medicines for high blood pressure are a common cause of dry mouth, and half of the people in the label’s trials had high blood pressure [1][10].

The label: no dry-mouth entry, but a fluid warning

Foundayo™ is a once-daily orforglipron tablet, a GLP-1 receptor agonist whose label does not call it a peptide [1]. Dry mouth is not among its listed reactions. The relevant warning is section 5.4: kidney injury, sometimes needing dialysis, has been reported with GLP-1 receptor agonists and with Foundayo™, mostly in people who became dehydrated after nausea, vomiting or diarrhoea [1]. The label asks prescribers to monitor kidney function in people with such reactions, especially when starting and raising the dose [1].

What the registries recorded

The ATTAIN weight trials only posted events affecting at least 5% of a group, so dry mouth is not visible there. The trials below published every event. They used trial doses of orforglipron in type 2 diabetes, a condition MedlinePlus itself lists as a cause of dry mouth [10].

Reading the registry entries

  • Dry mouth was rare everywhere, at most one person per group of 130 to 240, and dapagliflozin had a case too [6].
  • Dehydration entries were also few, but they appeared on orforglipron and not on placebo in the two double-blind trials [5][7], which fits the direction of the label’s kidney warning.
  • Some dehydration events were classed as serious: one in ACHIEVE-1 and one in the ATTAIN-2 weight trial, both on orforglipron [5][11].
  • Counts this small cannot give a rate, only a sense that dry mouth is uncommon and that fluid loss is the effect to watch.

How Foundayo™ might leave your mouth dry

Most clearly supported: fluid loss. Nausea, vomiting and diarrhoea affected a large share of people on Foundayo™ in the trials, more so during dose escalation [1]. MedlinePlus lists not drinking enough because you feel sick or nauseated as a cause of dehydration, and a dry or sticky mouth as a sign [9].

Plausible, not measured: eating and drinking less overall. The label says Foundayo™ lowers food intake through reduced appetite [1], and food and drinks are part of daily fluid intake, but no Foundayo™ study measured fluid intake or saliva.

Related, not the same: dysgeusia, a distorted taste, affected 0.9% on the tablet [1]. MedlinePlus lists trouble tasting among symptoms of dry mouth [10], so the two can feel linked.

Other common causes

MedlinePlus and NIDCR list many reasons the mouth becomes dry [10][12]:

  • Medicines, especially some for high blood pressure, depression and bladder control; hundreds of medicines can reduce saliva.
  • Diabetes, Sjögren’s disease and HIV.
  • Radiation to the head and neck, chemotherapy and immunotherapy.
  • Nerve damage from a head or neck injury.
  • Breathing through the mouth, and alcohol, tobacco and other substances.
  • Nervousness or stress, which can dry the mouth for a while in anyone.

What general sources suggest

MedlinePlus suggests drinking plenty of water, limiting caffeine, chewing sugarless gum or sucking sugarless sweets to stimulate saliva, avoiding spicy and salty foods, avoiding alcohol and tobacco, using a humidifier at night and choosing a mouthwash made for dry mouth [10]. Because saliva protects teeth, it advises brushing and flossing and seeing a dentist regularly [10].

NIDCR says dry mouth is not a normal part of ageing and that anyone who thinks they have it should see a dentist or doctor to find out why [12]. If a medicine is the cause, MedlinePlus notes that a provider or dentist may suggest changing the dose or the medicine [10]; that is a decision to make with them, not alone.

For the stomach side effects themselves, the Medication Guide asks you to drink fluids and to tell your provider right away about nausea, vomiting or diarrhoea that does not go away [2].

Points specific to Foundayo™

The label pairs the dehydration warning with the start of treatment and each dose increase [1]. Foundayo™ is raised no sooner than every 30 days [1], so the first days after a step are a sensible time to pay attention to thirst, urine colour and how often you drink.

Foundayo™ can be taken with or without food [1], which means a meal is not needed to take it. Pairing the daily tablet with a glass of water is a habit some people find useful, though the label sets no rule about water.

Questions

Does Foundayo™ cause dry mouth?

It is not listed in the label [1]. In trials that recorded every event, dry mouth affected at most one person per group, including on comparison drugs [5][6][7].

Could my dry mouth be dehydration?

It can be. MedlinePlus lists a dry or sticky mouth, thirst and dark urine among signs of dehydration, and the Foundayo™ label warns that stomach side effects can lead to it [1][9].

Why does my prescriber want to check my kidneys?

The label asks for kidney monitoring in people whose side effects could cause fluid loss, especially when starting and raising the dose [1].

Is it my blood pressure tablets?

Possibly. NIDCR names blood pressure medicines among those that often cause dry mouth [12]. Do not stop any of them without talking to your provider.

Does chewing gum help?

MedlinePlus suggests sugarless gum or sugarless sweets to stimulate saliva [10].

Should I see a dentist?

NIDCR advises seeing a dentist or doctor if you think you have dry mouth, and MedlinePlus notes that dry mouth raises the risk of tooth decay [10][12].
