Where dry mouth is, and is not, listed
Because each regulator uses different cut-offs, it is worth checking all three documents and one related label before drawing a conclusion.
| Source | Who was counted | Dry mouth |
|---|---|---|
| Wegovy® US label, Tables 3–5, text and post-marketing list [1] | Adults on 2.4 mg and 7.2 mg; adolescents | Not listed |
| Wegovy® EU SmPC section 4.8 [2] | Adults, STEP 1–4 pool and post-marketing reports | Not listed |
| Wegovy® Canadian monograph, Table 4 (≥1%) and section 8.3 [3] | Adults, STEP 1–3 | Not listed |
| Saxenda® (liraglutide 3 mg) US label, Table 2 [4] | Adults in weight-management trials | 2.3% vs 1.0% on placebo |
What reports and case studies describe
A study of the FDA Adverse Event Reporting System looked for ear, nose and throat problems reported with GLP-1 medicines until the end of 2023. For semaglutide, it found statistically significant signals for dry mouth, altered taste, loss of smell and Bell’s palsy. [5] The analysis covers every semaglutide product, not only Wegovy®, and the authors rate it as low-level evidence: a disproportionality signal means a problem was reported more often than expected, not that the medicine caused it.
A case series from Saudi Arabia described three women, average age 34, who had used semaglutide for weight loss for about 11 weeks and developed marked dryness with little saliva. After other causes were ruled out, they were diagnosed with semaglutide-associated hyposalivation; with stopping the drug, a saliva-stimulating medicine or symptom care, saliva flow recovered. [6] Three cases cannot tell us how often this happens.
Three possible routes
- Supported by the label: nausea, vomiting and diarrhea, which are most frequent during dose escalation, can cause fluid loss. [1] MedlinePlus lists a dry or sticky mouth and thirst among the first signs of mild dehydration. [8]
- Plausible but not measured for Wegovy®: the Canadian monograph reports decreased appetite in 9.3% vs 3.0% on placebo, [3] and eating less can also mean drinking less, since food and mealtimes supply part of daily fluid. No study has tested whether this explains dry mouth on Wegovy®.
- Raised by case reports: in the case series, the glands produced less saliva once other causes had been excluded, [6] but how semaglutide might do this was not established and no controlled study has tested it.
Common causes beyond the injection
The NIH lists hundreds of medicines that reduce saliva, especially some medicines for high blood pressure, depression and bladder control, as the most common cause of dry mouth. Diabetes, Sjögren’s disease and HIV, cancer treatment, nerve damage and the use of tobacco or alcohol are other causes. [7][9] Nervousness and stress can cause a temporary dry mouth too. [9]
People starting Wegovy® often take medicines for blood pressure or diabetes, so a full medicine review is a sensible first step. The NIH also stresses that dry mouth is not a normal part of ageing. [9]
Self-care the NIH describes
MedlinePlus and the National Institute of Dental and Craniofacial Research suggest drinking plenty of water, sipping water or a sugar-free drink with meals, chewing sugar-free gum or sucking sugar-free sweets to stimulate saliva, limiting caffeine, avoiding tobacco and alcohol, using a humidifier at night, and using a mouthwash made for dry mouth. Spicy or salty food can sting a dry mouth. [7][9]
Because saliva protects teeth, both sources stress brushing, flossing and regular dental visits. [7][9] A dentist or doctor can also suggest saliva substitutes. [9] Wegovy®’s own guidance on fluids is in the Medication Guide, which explains that diarrhea, nausea and vomiting can dehydrate you and that drinking fluids helps reduce that risk. [1]
