Switching from Zepbound to Wegovy: what the labels say

This page does not explain how to switch. It sets out what the two US labels say on the subject, and how PepLabo can keep both medicines in one record while your prescriber manages the change.

What the labels contain

  • Zepbound®: says combining it with other tirzepatide products or any GLP-1 receptor agonist is not recommended. No section on switching to another medicine [1].
  • Wegovy®: says using it with other semaglutide products or any other GLP-1 receptor agonist is not recommended. Its switching section (2.5) covers only Wegovy® injection and Wegovy® tablets [2].
  • The Wegovy® starting dose and steps (section 2.2) are written for anyone starting Wegovy® [2].

Why there is no dose conversion

The two medicines have different active ingredients, and each label lists its own starting dose and steps for people starting it. No label turns a dose of one into a dose of the other, so a conversion table found elsewhere is not something the labels support. Where to start on the new medicine is set by your prescriber.

Timing facts from the labels

Each label’s own numbers from section 12.3 and section 2. These are timing facts, not a switching schedule, and they do not turn one dose into another.

Switching the other way

Moving from Wegovy® to Zepbound® is not covered by either label [2][1]. The Zepbound® starting dose (section 2.1) is written for anyone starting Zepbound®; where a person coming from semaglutide begins is your prescriber’s call.

Questions to bring to your prescriber

  • When should the last Zepbound® dose be, and when the first Wegovy® dose?
  • Where on the Wegovy® steps will I start, and when is the next step reviewed?
  • Is any surgery or sedation planned? The Wegovy® label has a warning about it (section 5.10) [2].
  • Which side effects should I report, and how quickly?

Questions

Can Zepbound and Wegovy be used at the same time?

Both labels say combining them with another GLP-1 receptor agonist is not recommended [1][2]. Overlap or gaps during a change are for your prescriber to decide.

How do I keep both medicines in PepLabo?

Pause or finish the Zepbound® program and add Wegovy® as a new one. The free version keeps up to two active programs; past doses stay in your history either way.
